Download application
here or call PCC
for one.
- Mail the following to PCC:
- a completed
application/registration form
- payment-in-full for the courses
you have chosen
- two letters of reference from
sources familiar with your fitness expertise and/or character
- the prospective student questionnaire
- the signed Release and Indemnity
Agreement
- Watch the mail for your confirmation
letter. Upon satisfactory completion and acceptance of the admissions
requirements, a letter will be mailed to you confirming your acceptance
into the program.
Cancellation Policy:
If you cancel 21 days or more before the
start date of any course or workshop for which you are registered, you
will receive a full refund minus cost of materials and a $130.00
administration fee. No refunds will be given for any reason within 21 days
of the start date of the class. Rescheduling is allowed only once with a $70.00 rescheduling fee. There are no refunds or rescheduling after the start
of any course or workshop.
